Role: The post holder will enable clients who have been discharged from hospital or who are requiring assistance to remain living at home to regain their independence, manage their long-term conditions and support clients during periods of personal crisis.
Registered General Nurse
Role: To be responsible for the assessment of patient’s care needs, using a multi-disciplinary approach.
Health Care Assistant
Role: to assist the nursing and interdisciplinary team in the provision and delivery of prescribed programmes of patient care.
